Dear Parents and Prospective Students,

Thank you forshowing your interest to join usin Academic Achievement Open School as your Partner in Education.

Academic Achievement Open School is one of the branches of Academic Achievement Center LTD that offer education that prepares students for appearing exams of the Cambridge British curriculum - CAIE.

The center caters to all students without any discrimination on the basis of race, color, ethnicity, national origin, religion, creed or gender.

GUIDELINES:

1. All parts of this Application Form must be completed in BLOCKLETTERS.

2. Acceptance of this Application Form is not a commitment for Admission.

3. All prospective students will be required to appear for an entrance assessment. Details of the assessment will be made available at the time of submission of this application form.

4. Application fee of TShs. 30,000/-[Non-refundable]

5. The completed Application Form should be accompanied by the following:

• 3 recent passport size photographs of the applicant [1 to be pasted on this form]

• Copy of Birth Certificate

• Copy of the latest School Report Card

• Copy of Transfer Certificate for students Transferring from other schools

• Copy of Resident Permit or dependent pass for Non Citizens

• Copy of the Result Slip of the examination board

• Bring original of all the copies mentioned above for verification

6. All successful applicants accompanied by their respective Parents/ Guardians will be required to attend an admission interview.

7. For “A” Level, we offer only the following subjects : Mathematics(M), Geography(G), Physics(P), Chemistry(C), Biology(B), Psychology(Ps), Economics(E), Business Studies(Bs), Accounts(A) with the following combinations :
